Our Practice provides comprehensive evaluation and treatment of all types of back and neck problems. In addition to neck and back treatment
visits, we provide second opinion consultation, surgical opinion consultation, medical record review and all types of Workers Compensation
evaluation. Our staff members have been with us for many years and provide a seasoned team who will assist you in your treatment efforts.
Although expert surgery is always available as needed, the majority of our patients are assisted with simple conservative treatment
options.
Our office medical records are completed electronically and sent, without cost, to as many of your other physicians as you choose. The
office records are received by your other physicians on the day of your visit.
We have digital X-ray capabilities within the office and can provide you with a copy of your X-rays on a compact disc (CD),
upon your request.
Physical therapy is available at the office or by referral to a facility close to your home or office.
We are always happy to file your insurance for you and can assist in completing other forms upon request.

Our office is conveniently located near the IH 35 and Mockingbird Lane intersection in the twin Mockingbird Towers.
The East tower is located at the intersection of Mockingbird Lane and Elmbrook. All parking is free and Handicapped Accessible parking is
located on the upper level of the parking garage.

Appointments & Cancellations
All of our patients are seen by appointment only. Please call the main office number (214-630-7494)
to schedule your appointment.
We make every effort to maintain our office schedule. Emergencies do occur, making delays inevitable. We will make every effort to contact
you, at the numbers you provide, if a scheduling problem occurs.
If you are unable to make your appointment, please call our office within 24 hours of your appointment to avoid a charge for the missed visit.
Initial Visit
During your initial visit your medical history will be reviewed by our staff. A review of prior tests and a physical
examination will be made by the doctor. It is possible that the doctor will require further tests to determine the treatment options.
It is your responsibility to bring all films or CD’s of X-rays, MRI Scans, CT Scans, Discograms, Myelograms, EMG results and medical records
pertaining to your current problem. This information is crucial in determining your diagnosis and treatment. Without these, there is a
possibility that your appointment will be rescheduled.
Your co-pay is due at the time of your visit.

Prescriptions and Refills
Prescriptions and refills are issued during
OFFICE HOURS ONLY. After hours, the physician is unable
to access your medical records to determine if a prescription or refill is indicated.
Please allow 24 hours for the office to handle
